Hepatitis B Vaccine Information

After completing the Bloodborne Pathogen Training module, you must review the information sheet about Hepatitis B and complete the appropriate Hepatitis B Vaccine Waiver/Acceptance Form.

Your Bloodborne Pathogen training will not be complete until this form is submitted.

Vaccine Waiver/Acceptance Form Instructions

If you are affiliated with UT and working with bloodborne pathogens, UT offers the Hepatitis B vaccine to you at no charge.

  • If you have never received the vaccine and wish to get it, please complete Option A under the vaccine waiver/acceptance form.
  • If you have previously received the vaccine and/or do not wish to get it, please complete Option B on the form. If you decline the vaccine, you can still receive it anytime in the future.
  • If you have started the series, but only had 1 or 2 shots, you do not have complete protection. You do not have to restart the vaccine series, but you should finish the series for full protection.

If you elect to receive the vaccine, the Occupational Health Nurse will send you paperwork and orders to get the vaccine at UT Student Health Services. You must have the paperwork before you proceed to Student Health. Once you have the paperwork, you may go to Student Health any time between 8:00 am and 3:30 pm Monday-Friday. You do not need to make an appointment.
